Carmarthenshire County Council’s Looked After Children (LAC)

ELIGIBLE APPLICANTS INCLUDE:

1. Carmarthenshire County Council Foster Care Families / Looked after children (LAC) including:

• Foster Carers
• Foster Carer's Children
• Foster Children (Long & Short Term)
• Respite Placement
• A child can be looked after up to age of 18

2. Carmarthenshire County Council Care Leavers

A care leaver can be between the age of 16 and 25

Discounts Include:

1. Foster Care Families / Looked after children (LAC)

• Free Swimming during public times.
• Free Induction to Actif Fitness Suite (16yrs and over only).

Care Leavers

• One year Platinum membership (per individual care leaver), this membership entitles the care leaver to umlimited access to all Actif Leisure Centre gyms, swimming pools and fitness classes.
• Free Induction to Actif Fitness Suites (16yrs and over only).
• Free Swimming during public times.

How to Apply

1. Foster Care Families / Looked after children (LAC)

a) The Supervising Social Worker (SSW) will send out an application form and guidance notes to all registered foster carers.

b) On receipt of completed forms and photos from foster carers, the SSW will check the forms for accuracy, fill out section B and email the completed application forms and photos to actif@carmarthenshire.gov.uk

c) The Actif Sport and Leisure Central Business support team aim to process applications within 10 days.

d) For information, an e-mail will be sent to the relevant SSW and participating leisure site to confirm that the application has been processed and the membership cards and vouchers are sent out as applicable.

2. Care Leavers:

a) The Personal Adviser (PA) will complete one application form per care leaver.

b) The PA will attach a paragraph and email the application form to actif@carmarthenshire.gov.uk

c) The Actif Sport and Leisure Central Business support team aim to process applications within 10 days.

d) For information, an e-mail will be sent to the relevant SSW and participating leisure site to confirm that the application has been processed and the membership cards and vouchers are sent out as applicable.

Completing the Forms

Foster Families, Short Term Foster Children / Respite Places:

a) The Supervising Social Worker (SSW) must complete the form and attach 2 passport size photos to each application. 1 if digital and being emailed.

i. Please add the maximum number of places (at any one time) to one Carers' application form.

ii. Place the number of approved places you hold in the box marked "Short Term Foster Children / Respite Places" on the application form.

iii. Do not include photos for Looked after children.

b) Email completed application forms together with photos to actif@carmarthenshire.gov.uk

c) The Actif Sport and Leisure Central Business support team aim to process applications within 10 days.

Membership Cards / RFID buttons or keyrings

• An RFID button/keyring (membership access card) will be issued per person.
• The RFID button/keyring must be shown at each visit to the chosen Leisure site to gain the relevant discount / free activity.
• Membership cards will expire after 12 months.
• Memberships may be renewed by attaining a new application form from the Social Care Officers and reapplying as above. Not applicable by individuals leaving care.
• Membership cards for Short Term Foster Children / Respite Places will not have a photo or name instead "Child Complimentary Pass' will be written in the name strip.

Vouchers for Free inductions

• A voucher for all applicants aged 16+ for a free induction will be sent out with membership cards.
• As with the membership card, vouchers will have an expiry date of 12 months.
• Inductions must be booked in advance by contacting your chosen Leisure Centre.
• To obtain a free induction the voucher must be handed in to reception prior to the induction.
